Acupuncture for Fertility: Taking Your Basal Body Temperature

What is your Basal Body Temperature (BBT)?

Your basal body temperature is your temperature when you first wake up in the morning, before you have gone to the bathroom, had sex, started a conversation, or done any other activity.

It changes throughout your cycle and tracking your temperature gives us crucial information regarding when and whether you are ovulating, how well your hormones are regulating a healthy and balanced cycle, and the likely health and viability of your eggs, among other things.

How Acupuncturists use your BBT

While western medicine, midwives, and Natural Family Planning techniques also rely on your BBT charts, in Chinese medicine (TCM) we rely on them even more, as they reveal a ton of information to us regarding your general organ’s and body’s state of balance from a TCM perspective.  

Once the body is balanced, it is rare that there should be a reason that a woman cannot conceive, and so as the weeks and months of treatment progress, we are looking for progress and improvement in the BBT charts.

We also use the charts to make our treatment much more precise and accurate, and so the sooner you can start gathering your BBT charts for us, the faster we will have that information to use in your treatment.

Some acupuncturists don’t use BBTs with their fertility patients, for very understandable reasons: Usually women struggling with fertility already have months or years of micromanaging their lives and being self-proclaimed “OCD” in relation to their bodies and fertility. So tracking yet one more thing could create so much stress that it defeats the purpose.

However, except in the most rare case, I’ve found that women actually come to enjoy the process once they get into the habit. There’s nothing they have to DO about the numbers apart from what they’re already doing, and the peace of mind by seeing the charts improve over time, brings solace before that positive pregnancy test comes.

How to take your BBT

While there’s a lot of information online regarding the BBT which I encourage you to look into, and wonderful apps that can help you chart and get the information to me easily and conveniently, here I briefly go over the things you need to be tracking to help your treatment succeed.

First, you will need to get a BBT thermometer. These are different from the regular thermometers that you can get at drug stores, and are necessary for accurate tracking. You used to be able to go into any drug store and buy one, but for whatever reason the distributers stopped carrying them so our patients just use Amazon.

Technique

Each morning, as soon as possible after waking, ideally after at least 3 hours of uninterrupted sleep, and as close as possible to the same time each day; take your temperature and record it either on a paper chart, or on a phone app.

I recommend taking it vaginally, as this is more accurate than orally. Regardless of the location at which you take it, it MUST be consistent, as the vaginal temperature is higher than oral, and it’s the changes of temperature that matter.

You will also want to pay attention to a few other things throughout the month, which you should also note on your chart:
  • Your cervical fluid. Our cervix naturally secretes fluids all month, and the consistency changes throughout your cycle. The quality and amount of your fluids gives us a lot of clues not only regarding when you’re most fertile, but what imbalances are likely contributing to your inability to conceive to this point.  Learning about your fertile cervical mucous, which begins about 6 days before ovulation, alerts you to your most fertile days long before your temperature has risen to indicate ovulation. You want those little sperm up there waiting for the egg to be released, as the egg lives only for 6-12 hours, while the waiting sperm can live for up to 5 days!
  • The position and texture of your cervix: The position of your cervix also changes through the month, hanging low inside the vaginal wall until before ovulation when estrogen surges, causing it to move higher into the canal. At ovulation time the cervix is also much softer, as the Os (opening) softens.
  • Breast changes: Some women notice no changes in their breasts throughout the month, but if you do, you will want to take note of that on the chart.
  • Any abdominal symptoms, such as pain or cramping around menses or ovulation time, bloating, etc.
  • Bleeding or spotting: Of course, note the first day of your period and each day you are menstruating, but also be sure to note the quality of the blood. Is it bright red or dark, are there clots, is it mucousy, etc. You may want to consider switching from tampons to pads for the duration of your treatment, as this way you’ll be able to see and note changes in your blood more easily. Also notice if you have any mid cycle spotting.
  • When you had intercourse: The app I recommend has a way to distinguish whether it was protected or not.
  • Any anomalies: Did you sleep in a different bed, such as a hotel on vacation? Did you stay up very late the night before or drink way more than usual? Were you sick or did you feel like you were fighting something? Did you use a different thermometer? Anything that is different from your normal routine should also be noted, as these things can alter your temperature. 

I highly recommend using a good app to log all this information, as not only is it more convenient than keeping a paper and pen by the bed, but you always have it with you.

NOTE: If you are taking your temperature 10 minutes or more outside the typical time, please enter that temperature into an online Basal Body Temperature Adjuster. Just put in the time you usually take your temp, the time you actually took your temp, and your temp reading that day. Click submit and it will adjust your temperature to a more correct reading for that day. I have found that having women use this gives us a much more accurate reading of their temperature patterns.

Other info about your BBT charts

As I mentioned, one of the  great thing about the BBT chart is that it can help a women see visually that her body is getting healthier and more balanced in ways that matter for her to get pregnant, before she actually conceives.

While other symptoms such as digestive issues, headaches, anxiety or depression, fatigue, etc should start improving as treatment progresses, I know that ultimately, what every woman who is here for fertility is looking for in order to gauge the success of the treatment, is a positive pregnancy test and a healthy baby!

A woman who’s generally pretty healthy and doesn’t have a lot of other symptoms to go by can use her improving charts and cervical fluid as a way to see that her treatment with acupuncture and herbs is doing something important and that she’s heading in the direction needed in order for conception to take place.

Western vs. Traditional Chinese Medicine/Acupuncture for fertility

In TCM, we believe that barring specific issues such as prior surgeries, tubal blockages or other structural abnormalities, etc. there is no reason why women of the right age should not be able to conceive.  

Western medicine doesn’t foster health and balance, but instead uses extreme and often harsh methods to try to circumnavigate or mask the symptoms resulting from that poor health; or to force the body to do something it can’t naturally do because it’s so out of balance, such as conceiving.

Because western medicine is so extreme in its techniques, we as acupuncturists are often hard pressed to convince women that simply and gently “balancing” their body with acupuncture, herbs, and potential lifestyle changes; can be enough to solve their infertility problems.

However, a balanced body rarely has problems conceiving, and since a balanced body is reflected in a balanced BBT chart, if we can get your chart balanced, then there should be no reason why a woman shouldn’t successfully conceive.  

But, this doesn’t happen overnight

Because we aren’t going in with harsh drugs and techniques to FORCE pregnancy NOW, but rather are gradually and naturally creating a healthy internal ecosystem where a body will naturally get pregnant on its own without invasive measures, we need some cycles to see the improvement.

Most women who successfully conceive naturally with Chinese medicine do so within 6 months or so. Do not take a negative pregnancy test during that time to mean the treatment has been ineffective! Some changes in the BBT charts need a few cycles to kick in, depending on the imbalances and how deep seated they are.  

Women who have been on long courses of hormones such as birth control pills and/or rounds of IVF meds usually need more time to restore balance. Your reproductive health is a function of your overall health, and so the more we can improve all your parameters of health, the closer you are to that positive pregnancy test.

When should you delay attempts at pregnancy?

If there are more severe problems that indicate to us that it will be a longer haul or that you have a high risk of miscarriage, I may recommend NOT trying to conceive for a specific length of time while we work to restore health, vigor, and balance.

This will likely result in much easier conception once you do start trying again, but it will also likely mean a much healthier egg and embryo and therefore a much stronger constitution for your much awaited baby.  

For example, if a woman comes in who is not ovulating at all, and after a couple months of treatment she begins to ovulate but the chart shows that ovulation is happening really early, it is better to wait to try to conceive until ovulation is happening at the correct time. This is because those early releasing eggs will not be as fully developed as possible, which affects the health of the embryo and pregnancy.  Western ART techniques do not take these things into consideration but just force pregnancy at all costs, without first preparing the body for a healthy pregnancy and fetus.

What if natural fertility treatment with acupuncture doesn’t work?

Studies continuously show that women who use Chinese medicine have a significantly higher success rate with IVF, so if it did get to the point of resorting to ART the improvements in health and balance through Chinese medicine were not a waste, as they can save months of stress, prevent unnecessary repeated invasive drugs and procedures, as well as the resulting tens of thousands of dollars.

So while tracking your BBT is admittedly a big pain in the you-know-what, this simple chart gives us so much information that it can eliminate the need to start off with expensive tests, and often entirely prevents the need for any further ART.
